Community presence and participation, in ways that are age appropriate and make for valued members of society.

SUPPORT INTERVENTION APPROACH
All services are based on the recognition that people with a disability are individuals with rights, needs, and aspirations, who are members of families and of the wider community. Support Services are aimed at assisting people to live as valued members of the community and are based on an Individual Support Plan Model. Lifestyle Support Workers employed provide assistance to people receiving Lifestyle and Mobile Support Services in the client's home and around their local communities. Lifestyle Support Workers work in small teams and on a one to one basis and report through the Client Services and Lifestyle Support Facilitators.
